Study, you will receive a Free copy of your Personality
Profile Report. To Participate in the Study, you do NOT need to be a
business owner or manager seeking to use testing for your company, as this Study
is not for marketing purposes. You will be contacted to verify your eligibility
and some applicants will be spot-checked by telephone for control purposes.

If you choose to be part of this study,
you will take the following steps:
You will be linked to a Study
Application Form which you must fill out in its entirety.
The form will be reviewed by the Study
Administrator for inclusion in the Study. Minorities and people of all age
groups and job types are encouraged to apply, however the Study
Administrator may reject some applications if too many responses come from
any group, e.g. college students, unemployed individuals, etc. The Study
Administrator reserves the right to reject any or all requests without cause
or notice or to withdraw this offer at any time.
If your request is accepted, you will
be e-mailed the address of a special Internet site to visit and an
Authorization Code to use when you visit that site. Once accepted for this
Study, your name will not be used, rather a unique ID Code you choose will
be used instead in order to assure privacy and security of any further
information you provide.
When you visit the site where you will
take the Personality Profile Test, you will again be asked to fill out
information. After submitting the information completely, a Free copy of
your Personality Profile will be sent to you by E-mail. A copy of Adobe
Acrobat Reader will be
required to view your Free Report.
When you receive your Free copy of the
Personality Profile Report, you will be given another Internet site to visit
in order to provide comments and feedback about the Personality Profile
Report you received.
Upon completion of the follow-up
report, you may receive a short follow-up phone call from the Study
Administrator if there are any issues you raised that Hire Success would
like to address. Your comments are always welcome and appreciated.

You acknowledge you are at least 18 years or
older and are a resident of the United States or Canada.
You agree to provide the Hire Success Study
Administrator with true and accurate information about you that will be used
exclusively in this Study, and understand that no personal information will
be released to any other organization for any purpose and no information
will be used for marketing or solicitation purposes.
You agree to fill out a Post-Testing interview
form on-line after you have received your Free Personality Profile in order
to provide comments and feedback about the Test Results.
You understand that you may be contacted by
the Study Administrator to verify your eligibility and/or to discuss your
comments after receiving your Free copy of the Personality Profile.
You agree that the receipt of your Free
Personality Profile Report will be full and complete compensation for your
participation in this Study.
You agree keep your report confidential or, if
you make your report available to any other person or entity, that you will
hold Hire Success, and its parent company Decision Support Technology,
harmless for any consequences or damages that may result from such person or
entity seeing your report

household will be eligible for this study.

participants in any age group, or other demographic group are needed for
